About
High-performing teams are rarely accidental. They are the result of conscious effort to overcome inherent human behavioral challenges. This diagnostic tool, inspired by Patrick Lencioni's "Five Dysfunctions of a Team" model, quantifies the abstract elements of group dynamics: Trust, Conflict, Commitment, Accountability, and Results.
Unlike simple polls, this tool calculates a weighted index for each dysfunction. It features a "Multi-User Simulation" mode, allowing a leader to input estimated data for anonymous members to generate an aggregated "Health Heatmap." The output is not just a score but an actionable workshop agenda tailored to the specific toxic traits identified in the analysis.

The team score S is derived from the summation of Likert scale responses (1 to 5), normalized to a percentage.
