About

The Sequential Organ Failure Assessment (SOFA) score is a mortality prediction score that is based on the degree of dysfunction of six organ systems: respiratory, coagulatory, hepatic, cardiovascular, neurological, and renal. It is the operational standard for defining Sepsis under the Sepsis-3 international consensus (2016).

This tool allows for the rapid calculation of the score, handling the complexity of vasopressor dosage logic and PaO2/FiO2 ratio computations. A change in the SOFA score of 2 or more points consequent to an infection defines sepsis. The calculator provides dual-unit support (mg/dL and μmol/L) for international usability.

Formulas

The score ranges from 0 to 24. Points are assigned for worst values in 24 hours.

SOFA = Score(System0-4)

  • Respiratory: Based on PaO2/FiO2 ratio (mmHg).
  • Coagulation: Platelet count.
  • Liver: Bilirubin level.
  • CV: Mean Arterial Pressure (MAP) or Vasopressor dose (dopamine, epinephrine, norepinephrine in μg/kg/min).
  • CNS: Glasgow Coma Scale (GCS).
  • Renal: Creatinine or Urine Output.

Frequently Asked Questions

The CV score prioritizes the most potent vasoactive agent. If the patient is on Norepinephrine, that takes precedence over MAP. The score looks for the highest point value met. Doses are in mcg/kg/min.
For patients not on mechanical ventilation, SpO2 indices can sometimes be used as surrogates, but the official SOFA validation relies strictly on arterial blood gas (PaO2). This calculator requires PaO2.
